Call for More InformationOur social work staff provides supportive services to individuals as they participate in care. All Unity Wellness Center clients participate in an initial intake process, where the client and social worker collaborate to identify current strengths and needs. As needs are identified, the social worker will assist you by linking you to medical and community resources. The social work staff supports you by providing education regarding their current care options.
Case Management Services:
- HIV and Primary Medical care
- Alabama Drug Assistance Program (ADAP) for HIV medications
- Patient Assistance Programs for other prescribed medications
- Local charity care programs
- Mental Health referrals
- Substance Abuse Treatment referrals
- Dental care referrals
- Short-term rental and utility assistance
- Post-Test and Secondary Prevention Education
- Additional community referrals
- Transportation
To begin receiving HIV medical and supportive services, please contact Unity Wellness Center to schedule your initial intake appointment. In order to be eligible for an intake you must have documentation showing a positive HIV status and the below listed items.
- Picture ID
- Social Security Card
- Medical Insurance Cards (If you are insured)
- Proof of Alabama address
- Proof of income (If applicable)
- All current medications
