Please print and complete the following form before your first visit to Endocrinology & Metabolism of East Alabama.

Please allow 24 hours for us to process prescription refill requests. Remember to bring your glucose monitor with you to your appointment. New patients should arrive 30 minutes prior to their scheduled appointment in order for us to create a patient record.
